💻Patient Portal
Once we have processed your information, we will be sending you an email with information on how to access your Freddie Patient Portal. You will need to finish setting up your account by setting up a password using the following link: https://app.gofreddie.com/password/forgot.
This link will also be included in your initial welcome email from Freddie! Just input your email and you’ll receive a link to your email inbox to set your password.
Since we are in the pilot phase of our HIV Treatment program, some of the information in the Patient Portal is geared towards our services for PrEP so there's a few things to be aware of:
- The Clinical Messages and Lab Requisitions will be the main items we'll be using during this pilot.
- Please do not book any appointments via the Patient Portal. If you wish to book an appointment with a Freddie clinician, please reach out to us and we can support you with booking an appointment! (include contact support link)
- All other menu items such as the Prescriptions & Medications can be ignored for now.
- The patient portal allows you to securely communicate with your clinician outside of your appointment, and also view and download your requisitions for lab work.
- After you set your password, you can sign into the patient portal anytime with your email and password using this link: https://app.gofreddie.com/

🧪Lab Testing
Initial Lab Testing
Before your initial consultation, we will send you a lab requisition. We encourage you to complete your lab testing prior to your appointment, as this helps your clinician review your results and provide a more comprehensive consultation.
If you are unable to complete your testing beforehand, that’s perfectly okay — your appointment can still proceed as scheduled. Your clinician will discuss next steps and guide you through the steps to complete your lab testing.
Note that if you have an in-person appointment at PWA, you may not receive your lab requisition before your appointment. Your clinician can typically complete your lab testing during your in person appointment if needed!
Follow-Up Lab Testing
Follow-up lab testing is an important part of your ongoing care. These tests help ensure that your medication is working as expected and allow us to monitor your overall health. Once your results are received, our team will notify you, and when all results have been fully reviewed, your clinician will follow up with you through a clinical message. We’ll also inform you when your next lab testing is due via clinical messaging in the Freddie patient portal, so you never have to worry about missing it.

Important Notes:
- Plan to complete lab work Monday–Thursday.
- Download your lab requisition in the Freddie Patient Portal: After logging in, click the Lab Requisitions tab on the left-hand side of the page.
- You will receive an email notification whenever a new requisition is available and uploaded to your portal.

- Remember to bring your health card or government documentation to your lab appointment as well as your lab requisition
🪪If you have IFHP coverage:The lab will need a copy of your IFHP paper document (not just your number). Bring the paper document with you to the lab or bring it to your appointment at PWA. Upload it to your Freddie patient portal (instructions here). A photocopy or scan of your document is okay. It doesn’t need to be in colour.
📃If you received a Vitalink Requisition: Please ensure that you bring your Vitalink lab requisition along with the Vitalink letter that we will provide to you. Present both documents to the lab technician when completing your tests. Providing the letter will ensure that you are not charged for your lab testing.

Initial Appointment
Your initial phone appointment will be with one of our expert clinicians, who will review your health history and answer any questions you may have.
- PWA Patients: Initial appointments are scheduled in person or virtually (phone appointment)
Follow-Up Appointments
Follow-up appointments are required to maintain your prescription and ongoing care.
- PWA Patients: Follow-up appointments are typically in-person, but you have the option to complete them virtually if you prefer.
Need to book a new appointment or reschedule an appointment?

💊Prescriptions
The clinical team will issue your prescription according to where you are in your care journey. We have reminders in place to ensure you receive your next refill on time. Once your prescription has been issued, you’ll receive a clinical message through your Freddie patient portal to confirm that it’s ready.

🥼Partner Pharmacies
If you choose to use our partner pharmacy, your prescription will be sent directly to them. They can deliver it to you, or you can pick it up if you live nearby.
