The 519 Donation Campaign

This 519 donation campaign (the “Campaign”) is sponsored by PurposeMed Inc. (“PurposeMed”). The Campaign begins on May 1, 2024, at 12:00 a.m. Pacific Time (“PT”) and ends on May 31, 2024, provided it’s not terminated earlier by PurposeMed (the “Campaign Period”). Participation in the Campaign constitutes your full and unconditional acceptance of, and agreement to be legally bound by, these Campaign rules (the “Campaign Rules”).

  1. ELIGIBILITY:

To be eligible to win, a person must: (a) be a resident of Canada (excluding Québec), (b) have reached the age of majority in their province or territory of residence, and (c) be a patient of PurposeMed who has been directly invited by PurposeMed to participate in the Campaign.

  1. HOW TO ENTER:

No purchase is necessary. Entries that are incomplete, irregular, have been submitted through incorrect means, or do not conform to or satisfy any condition of these Campaign Rules shall be disqualified. Entrants acknowledge and accept the decisions of PurposeMed, or those acting on behalf of PurposeMed, as final and binding, without right of appeal, in all respects, including, without limitation, as to eligibility and/or disqualification of entries.

Entry: To participate in the Campaign, entrants must use the link provided by PurposeMed to complete the self-assessment questionnaire. 

Limit: one entry per person.

  1. DONATION:

DONATION(S): For every self-evaluation questionnaire completed by an eligible participant during the Promotion Period, PurposeMed will donate CAD$1 to The 519 (“Donation”). 

GENERAL: Donations must be accepted as provided and are not transferable or refundable. No substitutions except at PurposeMed’s option. PurposeMed reserves the right, in its sole and absolute discretion, to substitute the Donations or a component thereof with a Donation of equal or greater value.

  1. Participant Obligation:

Participants agree to provide accurate and truthful information in the self-evaluation questionnaire. PurposeMed reserves the right to verify the validity of questionnaire completions and to disqualify any participant providing false information.

  1. General Conditions:
    1. In consideration of the opportunity to enter the Campaign, each entrant, by entering the Campaign, grants to PurposeMed a paid-up, unlimited, irrevocable, non-exclusive, perpetual, royalty-free, sublicensable, transferrable and worldwide license to use its entry (including the answers in the self) for any purpose.
    2. PurposeMed reserves the right, at its sole discretion, to disqualify any individual (and all of his or her entries) who tampers with the entry process.
    3. If any taxes or levies apply to the Donation, it shall be the sole responsibility of the Campaign Participant to fulfill any associated tax obligations. PurposeMed assumes no liability for the Participant's tax liabilities, and the Participant agrees to indemnify and hold PurposeMed harmless in this regard.
    4. This Campaign is subject to all applicable federal, provincial, and municipal laws and is void where prohibited by law.
    5. By entering the Campaign, entrants agree to indemnify and hold PurposeMed harmless from any claims, liabilities, damages, or expenses arising in connection with the Campaign. PurposeMed will not be liable for any loss or damage arising from participation in the Campaign or from the participant's failure to comply with these Campaign Rules. Entrants also release PurposeMed from any and all liability related to their participation in the Campaign.
    6. PurposeMed reserves the right to cancel, suspend, withdraw or amend this Campaign in any way, without prior notice or obligation.
    7. PurposeMed accepts no responsibility for loss, damage or claims caused by or resulting from the Campaign or a Donation.
    8. PurposeMed will not be liable in the event of an error, technical problem, computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure or any other cause beyond the reasonable control of PurposeMed that interferes with the proper conduct of this Campaign as contemplated by these Campaign Rules.
    9. PurposeMed reserves the right, in its sole and absolute discretion, to require proof of identity and/or eligibility (in a form acceptable to PurposeMed – including, without limitation, government-issued photo identification): (i) for the purposes of verifying an individual’s eligibility to participate in this Campaign; (ii) for the purposes of verifying the eligibility and/or legitimacy of any Entry entered (or purportedly entered) for the purposes of this Campaign; and/or (iii) for any other reason PurposeMed deems necessary, in its sole and absolute discretion, for the purposes of administering this Campaign per these Campaign Rules. Failure to provide such proof to the satisfaction of PurposeMed in a timely manner may result in disqualification in the sole and absolute discretion of PurposeMed. The sole determinant of the time for the purposes of a valid Entry in this Campaign will be the Campaign server machine(s).
    10. PurposeMed respects your right to privacy. By entering this Campaign, each entrant expressly consents to PurposeMed, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry for the purpose of administering the Campaign, improving PurposeMed’s services, and in accordance with PurposeMed’s Privacy Policy (available at: https://www.purposemed.com/privacy-policy) unless the entrant otherwise agrees to receive further communications from PurposeMed.
