When your Freddie clinician indicates lab testing is needed, we will send you an electronic copy of your lab requisition to your Freddie patient portal. You will need to present your lab requisition to the lab technician when you go in to complete testing. Below are some options for this:
1. Print your lab requisition:
- Navigate to your Freddie patient dashboard
- Click the "download requisition" button
- Find the "print" option on your device
- Ensure your printer is connected to your device
- Print your lab requisition and bring it with you to your lab appointment!
If you do not have access to a printer, see option 2.
2. Email your requisition:
- Navigate to your Freddie patient dashboard
- Click the "download requisition" button
- Download the PDF to your device
- Open your email account on your device and create a new email
- Attach the newly downloaded PDF copy of your lab requisition
- Send email with PDF attachment to the confidential email address that the lab will provide you

3. Fax your lab requisition:
- Reach out to our Support team with a request to fax your lab requisition
- Please provide the name, address and fax number of your preferred lab location
- Our team will respond to let you know when your requisition has been faxed. Please allow up to 2 business days for your request to be processed
