If you’re taking PrEP (pre-exposure prophylaxis), there are a few tests we do before you start the medication and on a regular basis while you’re on it. These tests help keep you healthy and make sure PrEP is safe and effective for you. We also follow Canadian guidelines to determine which tests are needed. Here’s a breakdown of what we check and why:
HIV
PrEP is intended for HIV prevention, and its safe use requires an HIV-negative status. Prior testing is essential, as initiating PrEP while HIV-positive carries the risk of the virus developing drug resistance, which is why we always test first.
Syphilis
Syphilis is a sexually transmitted infection (STI) that can cause serious health problems if it’s not treated. Testing helps catch it early so you can get the right treatment quickly.
Creatinine (Kidney Function)
PrEP is processed through your kidneys, so we check kidney function (creatinine and eGFR) to make sure your kidneys are healthy and can handle the medication safely.
Hepatitis A, B, and C
Hepatitis A spreads through close personal contact, including sex or living with someone who has it, and it’s found in the blood and feces of people who have the virus.
Hepatitis B can be spread through bodily fluid such as blood, saliva, vaginal and seminal fluid.
Hepatitis C, like HIV, can be passed through sexual contact or blood-to-blood contact (for example, sharing needles). People at risk for HIV exposure are often at risk for these viruses too.
We only test for hepatitis A, B, and C at your initial visit. Vaccines for Hepatitis A and B aren’t required to start PrEP, but we strongly recommend them if your tests show you’re not immune. There isn’t a vaccine available for Hepatitis C.
Chlamydia and Gonorrhea (Urine)
These STIs are common and often don’t cause symptoms. Screening ensures we catch them early so you can get treatment and avoid long-term complications.
Chlamydia and Gonorrhea (Throat and Rectal Swab)
Swab testing for the throat and rectum is not required for your PrEP refills, but it can be done if you’d like more thorough screening. You can refer to this article for more information:: Swab testing with Freddie
