You can find your lab requisitions in Freddie’s secure patient portal. After you’ve logged in, click “Lab Requisitions” on the left-hand side of the page. These are sent to you automatically – you won’t need to ask us for them. Whenever a new requisition is available, you will be notified via email, confirming its upload to your personal portal. If you’ve told us about a preferred lab, we will also fax these directly to them as well as sending you the digital copy.
If not, we recommend printing out a copy to bring with you, as not all labs accept digital copies. You can also call the lab in advance to confirm whether or not they will accept a digital copy from your mobile device.
