You can find your lab requisitions in the Freddie patient portal. After you’ve logged in, click the Lab Requisitions tab on the left-hand side of the page.
Whenever a new requisition is available, you will be notified via email, confirming its uploaded to your patient portal.

It is important to note that we no longer automatically fax your requisition to the lab on your behalf.

Please note you will need to reach out to us each time you need your requisition faxed to the lab location of your choice.
When will I receive my lab requisition?
Please allow up to 1 business day for your initial lab requisition to be uploaded to your patient portal after you complete the intake questionnaire. If you need your lab requisition sooner, or if it is past the specified timeline, please reach out to our Support Team for assistance.
