You can find your lab requisitions in the Freddie patient portal. After you’ve logged in, click the Lab Requisitions tab on the left-hand side of the page. These are sent to you automatically – you won’t need to ask us for them.
Whenever a new requisition is available, you will be notified via email, confirming its uploaded to your patient portal.
If you’ve told us about a preferred lab, we will also fax your lab requisition directly to them as well as sending you the digital copy.
If not, we recommend printing out a copy to bring with you, as not all labs accept digital copies. You can also call the lab in advance to confirm whether or not they will accept a digital copy from your mobile device.
Please allow up to 48 hours for your preferred lab to receive a faxed requisition.
For your PrEP refill:
We will send your 3 month follow-up lab requisition when you have around one bottle remaining in your current prescription. You will first need to compete the self-assessment questions in the Dashboard section of your patient portal. After this, your lab requisition will be unlocked. You can then click "download requisition" to view & download your new lab requisition form.

If you have less than 20 pills remaining in your current prescription and you have yet to receive your follow-up lab requisition, please reach out to our Support Team for assistance.
After your initial consult:
Please allow up to 1 business day for your initial lab requisition to be uploaded to your patient portal. If you need your lab requisition sooner, or if it is past the specified timeline, please reach out to our Support Team for assistance.
